Enter our NEW Form-A-Lic Calendar Photo Contest!
We are creating a 2021 calendar featuring photos of beef cattle, dairy cattle, horses, and sheep enjoying Form-A-Lic, and we are looking for great photos to fill our calendar!
Please enter your photos using the form below by December 1, 2020. Twelve photos will be chosen to be included in the Form-A-Lic calendar. If your photo is chosen, you will receive a $25 Visa gift card. One photo will be chosen as the grand prize winner. The grand prize photo will win a $100 Visa gift card! All entrants will receive a copy of the Form-A-Lic 2021 calendar.
Limit of three photo entries per person.
Photos submitted for the Form-A-Lic photo contest may be used for promotional purposes such as advertisements, literature, websites, and more.
